What age range of patients does Seguin Family Medicine treat?
We see patients of all age groups, from infants to the elderly.
Are you accepting new patients?
We are accepting new patients at the current time. New Patient Forms must be completed prior to setting an initial appointment.
Do you have same day appointments?
Yes, we do. These are usually reserved for patients who have an acute illness. For less urgent needs, please schedule your visit ahead of time.
Scheduling and Cancellation Policies
If unable to keep your appointment, please call in advance to cancel and reschedule. Without notification of cancellation, another patient may be deprived of an appointment.
T better serve our patients, our office has adopted a "No Show Policy". An appointment is considered a "No Show" appointment if the patient fails to cancel the appointment at least 24 hours in advance. After (2) two "No Show" appointments within (1) one year, it is with our regret, that the patient will be terminated from our practice.

If you have an immediate medical need, you may call our office at any time. Calls placed after normal office hours will be directed to the on-call physician. The physician will decide on the best course of action, which may include a visit to an emergency room or an urgent care clinic for more extensive evaluation.
How do you handle prescription refills?
Prescription refills are typically taken care of the day we receive the refill request from your pharmacy. To avoid running out of medication, we suggest you contact your pharmacy for refills when you have 7 days of medication remaining. In most cases, we can renew medications without a new visit if you have been seen for this problem within the past year. The provider may, however, request that you be seen more frequently to allow for proper evaluation and documentation of your condition and your response to the medication.
How do I obtain test results?
Lab and radiology results are typically reviewed during your pre-scheduled lab appointment. Lab results can be very confusing and sometimes frightening to the untrained eye, therefore your physician will go over your lab results thoroughly with you. We will contact you by phone or by mail with your radiology results. If further treatment is necessary, upon receiving your results, we will schedule an appointment at that time.
We have a full service draw station in our office for your convenience. Some lab results are available immediately, such as pregnancy, urinary tract infections, strep throat, and influenza.
What if I have a question about my health?
Please call our office and the staff will take a message. Non-urgent messages will be relayed to your provider. Expect to receive a reply within 1-3 days unless your doctor is out of town. For urgent issues, your provider (or someone else if your provider is not available) will usually be able to get back to you the same day either between patient visits or at the end of the day. Replies to messages may be relayed to our staff who will call you with the details.
What insurance plans does Seguin Family Medicine accept?
We accept most major insurances. If you have any questions about your insurance, please feel free to contact our office. Payment for insurance co-pays is expected at the time of service. We accept cash, checks and major credit cards.
Do you have a lab or X-ray in your office?
We provide lab testing in our office. including tests for pregnancy, blood sugar, anemia, urinary tract infections, and Strep throat. We do not have X-ray facilities in our office but can order X-rays.
How do I have my records transferred?
If you will be coming to our practice as a new patient, it is often useful to get us a copy of your recent medical records from your last doctor(s). To get a copy of your medical record sent to us, please complete the "Request for Transfer of Medical Records" form (click on link below) and mail this form to your last doctor's office. Depending on the doctor's office, we will typically receive your records within a few days to three weeks time. You may want to contact our office 1-2 weeks after mailing the form to see if we have received your records. If not, it may be necessary for you to send another copy of the form or to call your last doctor's office to ensure a copy of your records will be sent promptly.

payment policy
Payment Policy
All self-pay patients are required to pay in full at the time of service. Patients with co-pays must pay their co-pay at the time the service is rendered. Once we receive the Explanation of Benefits from your insurance company, you will be billed for any balance that you owe. This amount is due in 30 days.
Accounts that are repeatedly ignored will be referred either to a collection agency or an attorney and you will be financially responsible for your outstanding balance and all legal fees involved. Should this occur, you may be dismissed from the practice and asked to find a new physician.
If a check is returned for insufficient funds, you will be charged an additional $30.00 on top of the amount of the check.
If you wish to suspend or terminate your treatment, any fees including reasonable fees as allowed by public health law for copying of medical records will be immediately due and payable.

Pre-Approval
Almost every insurance plan requires pre-approval for specialty care. Without specific approval, insurance carriers may not cover charges incurred for the unapproved visit. It is the patient's responsibility to obtain pre-approval from their insurance carrier. Certain insurance carriers may also require a specific referral from the patient's primary care physician.

Worker's Compensation
Seguin Family Medicine is not a participating provider with Worker's Compensation programs.
Motor Vehicle Accidents - Auto Insurance
All patients are personally and financially responsible for their medical care regardless of the nature of the injury or potential third party involvement. Patients often believe that the other party will pay their medical bills as the accident was “their fault”. Because of the individual nature and complexity of these cases, we will not accept a letter of protection from an attorney, and it is not possible for us to get involved in collecting from a third party. Seguin Family Medicine will not submit claims to your auto insurance carrier. You will be responsible for portions of your bill not paid by your insurance company. To expedite this procedure, you must provide us with the insurance company’s name and address, adjuster's name, phone number, date of injury and your MVA claim number at the time of your appointment.
Co-payments
A co-payment is an amount established by individual insurance plans and is usually specified on the insurance card. Co-pays are a required part of your contract with your insurance carrier and increase the cost of billing unnecessarily if not paid at the time of service. It is required that this amount be paid at the time of service.
Personal Balances
Any portion of your bill that is not covered by your insurance carrier becomes your personal responsibility. Seguin Family Medicine will bill individuals on a monthly basis and personal balances are due upon receipt of the statement. Payment terms may be arranged through the Business Office.

A Release of Information Authorization is included in your registration materials. This form authorizes Seguin Family Medicine to release information to your insurance provider as well as any health care provider involved with your current treatment. A new Release of Information Authorization should be completed any time your coverage changes and yearly thereafter to assure that your claims will be promptly filed and paid.

It can be hard for doctors and loved ones to know what kind of treatment you would want when you are too sick to tell them. Advance directives are legal documents designed to allow competent patients the opportunity to guide future health care decisions in the event that they are unable to participate directly in medical decision making.
The two basic types of advance directives are Living Wills and Power of Attorney-Health Care.
Living Wills
A Living Will, or "Declaration to Physicians," is a signed and notarized legal document in which you declare that in the event you have a terminal illness or injury, you want to die naturally and not be kept alive by extraordinary or artificial means.
Once your Living Will is executed, the original should be made part of your medical record. Give copies to family members and friends who are likely to be involved in your health care decision-making.
Power of Attorney - Health Care
A durable power of attorney (DPA) for health care is another kind of advance directive. A DPA is a legal document that states the person you have chosen to make health care decisions for you if you are unable to make those decisions for yourself. It becomes active any time you are unconscious or unable to make medical decisions. A DPA is generally more useful than a living will. However, a DPA may not be a good choice if you don't have another person you trust to make these decisions for you.
The person you choose to act on your behalf is called a health care "agent". Anyone of sound mind who is at least 18 years old may be designated as your health care agent except for your health care provider, or an employee of your health care provider, unless that person is your relative.
Upon completion of your Health Care Power of Attorney, give the original document to your physician. Give copies to your health care agent, your family members, and your attorney.
